The fifth stage has more altered sprites and portraits. The Hydra (Hidora) of the Japanese game fights differently from the one in the U.S. game. Kelbelos, the main boss, remains pretty identical in both games. The only difference is the character portraits between versions.

Lastly, you have the final stage! Remember Niou (Nioh), the main boss of the first level in the Japanese game? He acts as the mini-boss of the final level in the U.S. game. Remember Insector, the main boss of the first level in the U.S. game? You fight a boss called "Beruzebabu" (or Beelzebub) that is similar to Insector. The name "Beelzebub" has a deep meaning in the U.S. and that's why we got "Insector" instead. The main boss of the U.S. game is known as Muramasa. The main boss of the Japanese game is Satan. Muramasa and Satan are nothing alike. As a matter of fact, you have to fight Satan twice whereas Muramasa must only be defeated once.
